Medication Synchronization

Medication Therapy Management
  • Medication synchronization, also known as med sync or refill synchronization, is a service offered by Ezcare Pharmacy to help streamline the prescription refill process for patients. It involves aligning the refill dates of multiple ongoing medications so that they can be picked up simultaneously each month.
  • The process typically begins with a consultation between the patient and a pharmacist or pharmacy staff member. During this consultation, the patient lists their regular medications, including prescription, over-the-counter, and supplements. The pharmacist reviews the list and assesses the synchronization feasibility.
  • Once the medications are identified for synchronization, the pharmacist will work with the patient's healthcare provider(s) to adjust prescription durations, quantities, and refill dates to achieve synchronization. This may involve modifying prescription orders, extending or shortening medication supplies, or adjusting dosing instructions if necessary. The goal is to coordinate all the medications to be refilled simultaneously.
  • The synchronized refill dates are typically set to occur on a specific day of the month, such as the first or the fifteenth. The pharmacy will then prepare the medications and have them ready for pickup or, in some cases, deliver them to the patient's home. The patient is notified when their medications are ready for pickup or delivery, ensuring they have a continuous supply of all their medications without needing multiple trips to the pharmacy.
  • Medication synchronization offers several benefits to patients. It improves medication adherence by reducing the risk of missed or delayed refills. It also simplifies medication management, as patients only need to keep track of a single refill date for all their medications. This service can save patients and their caregivers time and effort and contributes to better overall medication management and health outcomes.
