- The process typically begins with a consultation between the patient and a pharmacist or pharmacy staff member. During this consultation, the patient lists their regular medications, including prescription, over-the-counter, and supplements. The pharmacist reviews the list and assesses the synchronization feasibility.
- Once the medications are identified for synchronization, the pharmacist will work with the patient's healthcare provider(s) to adjust prescription durations, quantities, and refill dates to achieve synchronization.
