The Med-to-Bed program at Ezcare Pharmacy is designed to provide a seamless transition from the Hospital Discharge to the patient's home. Prescription Coordination

Once a prescription is received from the hospital for a discharged patient, our highly trained pharmacists review the prescription for accuracy and potential drug interactions. Delivery or In-Store Pickup

We offer our patients the convenience of home delivery or in-store pickup options. Patients can choose the option that best suits their needs and preferences. Home delivery ensures patients receive their medications directly at their doorstep, saving them time and effort.

Medication Counseling

Our pharmacists provide one-on-one counseling to patients to ensure they understand their medications' purpose, dosage, and potential side effects. They address any questions or concerns the patients may have and provide helpful instructions on taking the medications properly.

Refill Management

Our Med-to-Bed program includes proactive refill management. Our pharmacists keep track of prescription refills and reach out to patients to ensure they have a continuous supply of medications. Collaboration with Healthcare Providers

We maintain open communication channels with hospital-discharged patients’ PCP or Specialists for any medication-related issues or to continue care promptly. Our pharmacists work closely with the providers to ensure medication regimens are optimized and appropriate for the patient's condition.
